POLICY

 REQUEST FOR SPEECH AND LANGUAGE SCREENING/TESTING

Speech and language services are provided to the district by the speech and language clinician. Teachers may initiate a referral or be asked to complete a referral form by the building principal or director of pupil services. These forms are available in the school office. The teacher must complete the referral form and submit it to the building principal or director of pupil services for approval. The teacher must sign each form as the person making the referral.

The principal initials the form to indicate that it is complete and approved, and forwards it to the speech and language clinician. At this point the clinician may request additional information from the classroom teacher. The clinician will secure parental consent for testing on district forms.

Building principals should keep a list of referrals made during each school year as well as a current list of all children receiving language services.
