PROCEDURE FOR EMPLOYEE LEAVE REQUESTS AND LEAVE UTILIZATION REPORTING
A careful and precise accounting of leave utilization by employees is important for both cost control and equity among employees. The administrators of Susquehanna Township School District are directed to utilize the following procedure for approving leave requests and recording leave utilization.
All leaves except sick leave and bereavement leave require pre-approval. An employee leave request form is part of this policy. This quadruplicate form is completed by the employee and forwarded to the supervisor. The supervisor makes a recommendation in the portion provided and forwards the form to the superintendent/designee for final approval. If approved, the form is distributed, as shown, to the business office, supervisor, and employee.
Immediately upon returning to work after any leave, each employee is required to complete the leave utilization form that is part of this policy. The completed form is turned in to the supervisor. If the leave has been pre-approved, the blue copy of the leave request form should be stapled to the leave utilization form.
Each supervisor is required to submit all completed leave utilization forms with payroll reporting data that is turned into the business office on a bi-weekly basis.
The business office submits completed leave utilization forms to the superintendent/designee for approval bi-weekly.
The business office keeps individual records of leave utilization. At the end of January and the end of June of each year a composite of leave utilization is provided to the superintendent in the following form.
Other Certified Personnel
This information is then submitted to the Board of Education.
